You can use this form to let us know if you would like someone to receive a death in service lump sum should you die while employed with Accenture.

Please note that the Trustees have discretion as to whom payments are made. However, they will take your wishes into account.

Therefore, please name who you would like to be considered by the Trustees as the recipient of any available lump sum should you die while employed with Accenture.
