We need some information
Your employer has arranged insurance cover with us that’s designed to provide protection for you as their employee, where financial help could be provided if a claim is made. To set up this cover we need to collect details about your health and activities, and your consent to:
- access your medical reports that may be needed to help us confirm our decision
We’ll only use the information provided by you on this form, your General Practitioner (GP) and any medical practitioner we may ask you for the purpose of managing and arranging this insurance cover.
