Who you’d like your benefits paid to - form

Confirm who you'd like your benefits paid to if you died by completing a 'nominate your beneficiaries' form. This will make it clear who you'd like your savings to go to if you still have money left in your pot at that time. The SAUL Trustee has discretion as to who receives the money but will take your wishes into account.

Serious-ill-health retirement application

To retire on serious-ill-health-grounds, the SAUL Rules say you must have a life expectancy of less than 12 months. To help the Trustee decide if you can retire, they need information about your health. Please use this form to give as much information as you can.
