Bereavement Notification

We are very sorry for your loss and understand this may be a difficult time.

This form has been designed to make notifying us of a bereavement as simple as possible. We'll ask for the information we need to process the notification.

Once completed, we'll confirm your request and any next steps.

To get started, we'll need some information about the person who has passed away.

Your Details

We'll now ask for some details about you, as the person submitting this request and notifying us of the bereavement.

 

Example: If your mother has passed away and you are completing this form, select "Son" (your relationship to the deceased), not "Mother".

 Spouse/ Civil Partner/ Partners Details

We'll now ask for some details about the spouse, if you do not know the infomation please continue to submit button. 

To enable us to fully process the notification, we will need some supporting documents, such as the death certificate. 

If you have these available, you can provide them now, or you can send them to us later once we have confirmed the next steps to you in writing.
